  • Form A1 (PDF, 438.6 KB): Application for assessment of academic qualifications – overseas qualifications - Original documents and fee required.
  • Form A2 (PDF, 417.7 KB): Application for assessment of academic qualifications – final or prospective assessment of studies other than studies with the Board.
  • Form A3 (PDF, 447.9 KB): Application for assessment of academic qualifications – incomplete or unaccredited qualifications, or qualifications completed more than five years ago.
  • Form P1 (PDF, 441.9 KB): Application for assessment of practical legal training qualifications – overseas qualifications services or courts administration.
  • Form P2 (PDF, 434.3 KB): Application for assessment of practical legal training qualifications – NSW government.
  • Form P3 (PDF, 439.5 KB): Application for assessment of practical legal training qualifications  - incomplete or unaccredited qualifications, or Australian qualifications completed more than five years ago.
  • Form R1 (PDF, 584.0 KB): Application for review of decision of the Academic Exemptions Sub Committee. Original documents and fee required.
  • Form R2 (PDF, 572.5 KB): Application for review of decision of the Practical Training Exemptions Sub Committee. Original documents and fee required.
  • Form - Application for extension of period of validity (PDF, 439.4 KB): Academic or PLT direction or exemption.
